Initial Consultation:

Your initial consultation will consist of comprehensive evaluation by Dr. Kang explaining in detail your diagnosis and possible treatment options. Surgery can be performed on the same day but a complex surgical procedure or medical history would require further workup prior to treatment. If you plan to go under IV sedation, an initial consultation as well as a physical exam is necessary prior to being put to sleep. Dr. Kang will explain the procedure in detail along with the sequence of treatment and review all imaging with you.

Please bring your referral slip, x-rays, and a list of medications you are taking to your initial consultation. At times, Dr. Kang will determine that additional imaging may be needed including a panoramic x-ray or 3-D imaging.

A valid identification, current medical or dental insurance card, and any other pertinent information will help our front office during this process. Please have your dentist or physician email any x-rays they have taken or if possible, you can pick them up and bring them with you.

If you are under the age of 18, a parent or legal guardian must accompany you.

During initial consultation, please provide the following information.

• Physician or dentist referral
• List of all medications you are presently taking
• Copy of all x-rays or 3-D imaging
• Medical or dental insurance information
